This is the online payment terms and conditions (terms) for Urology Care Alliance (UCA). This online payment service is provided subject to your compliance with the terms and conditions set forth below. Your use of this online payment service indicates your agreement to be bound by these terms and conditions.

These terms apply only to the UCA online payment service.  You certify you are an authorized user of the credit card account used in making payment.  You expressly agree that your use of this service is at your sole risk. You agree that you will not intentionally provide false information when accessing or using the Urology Care Alliance (UCA) online payment service. You agree to notify Urology Care Alliance (UCA) immediately upon your discovery of unauthorized use of your account and any other breach of security.

Urology Care Alliance (UCA) will not be liable for any damages of any kind arising from the use of this site, including, but not limited to, direct, indirect, incidental, consequential and punitive damages.


Urology Care Alliance (UCA) online payment service is provided to you to facilitate payment on your account. All transactions are considered complete upon receipt. You will receive an email confirmation once payment has been received. When you make a payment online, it will take up to 48 business hours for the payment to be applied to your account.

UCA will not retain any personal information. It will only be used for the current payment. If you wish to make future payments online you will need to re-enter the required information.

Urology Care Alliance (UCA) will not be liable for any deficiencies in the accuracy, completeness, availability or timeliness of payment information sent via this service.


All requests for refunds of credit balances are handled by the UCA Billing Office.  You may contact our Billing Office at 609-896-2950 and refunds will be reviewed on a case by case basis.


UCA respects and is committed to protecting your privacy.  We may collect personally identifiable information when you visit our site.  It is UCA’s policy not to sell your personally identifiable information to anyone.

Urology Care Alliance (UCA) takes appropriate steps to ensure data privacy and security, including the use of various hardware and software methodologies.  However, UCA cannot guarantee the security of any information that is disclosed online.  Our merchant provider uses Secure Sockets Layer (SSL) software that is the industry standard and among the best software available today for secure commerce transactions.  It encrypts all of your personal information, including credit card number, name and address, so that it cannot be read over the internet.  UCA does not keep credit card information. Once the online transaction is complete, it is no longer available. Your information is not stored for future payments.
